Contract Employee Benefits Summary
The workweek begins on Sunday and ends on Saturday. We are currently implementing our new electronic timecard system where employees enter their time online and managers can approve timecards online. A time card with a manager’s signature must be received before a paycheck can be released. Electronic timecards must be submitted by the end of the workweek. Payroll deadline for approvals is 10am on Monday.
Employees can sign up for direct deposit at the time of hire.
An employee may be eligible for state workers’ compensation benefits for job-related injuries. If an employee is injured while performing a job-related duty, the employee should immediately report the accident to his/her supervisor. If the injury results in absence from work, a letter from a doctor stating that the employee is physically well enough to return to work is required prior to, or on, the date of return.
Section 125 Cafeteria Plan
This IRS-approved plan allows Lighthouse employees to have weekly medical insurance premiums deducted from their paychecks pretax. Reducing taxable earnings leads to less taxes being taken out of the employee’s pay on a weekly basis.
Lighthouse Professional Services offers three quality insurance options for the health and well being of its employees. Our Blue Cross Blue Shield HMOs provide medical coverage for employees living in New England, while our PPO provides medical coverage for employees nationwide.
Blue Cross Blue Shield HMO Access Blue New England $2000/4000 Deductible
This HMO is offered to eligible employees living in New England who have been with Lighthouse for 90 days. Employees must maintain an average of 30 hours per week to become and remain eligible. This plan has the same excellent coverage as our most expensive plan. It has a lower premium in exchange for a higher deductible. The deductible is $2,000 for each member or $4000 per family.
Blue Cross Blue Shield Options Deductible II Plan
This HMO is offered to eligible employees living in New England who have been with Lighthouse for 90 days. Employees must maintain an average of 30 hours per week to become and remain eligible. Covered services include outpatient and inpatient care, prescription drug benefits, mental health and substance abuse treatment as well as a variety of other “Healthy Blue” programs from which to choose. The plan’s deductible varies from $0 for an individual/family up to $4000 for a family plan depending on the tier the provider is under. The following services are not subject to the deductible and copayments can range as low as $0: office visits including those for vision and hearing, preventive health services, prescription drugs and all mental health services. The benefit of this plan is the subscriber’s ability to control out-of-pocket costs.
Blue Cross Blue Shield Preferred Blue PPO Basic $2000/$4000 Deductible
Because the PPO network is nationwide, this plan is perfect for Lighthouse employees living outside New England. The PPO is offered to eligible employees who have been with Lighthouse for 90 days. Employees must maintain an average of 30 hours per week to become and remain eligible. Covered services include outpatient and inpatient care, prescription drug benefits, mental health and substance abuse treatment as well as a variety of other “Healthy Blue” programs from which to choose. The plan’s deductible is $2000 for an individual or $4000 for a family plan. The following services are not subject to the deductible and for the most part cost a copayment of $25 in network: office visits including those for vision and hearing, preventive health services and mental health services.

Altus Dental’s Altus Plus Dental Plan
Altus Plus is an affordable dental plan offered to employees who have been with Lighthouse for 90 days and average 30 hours per week. Preventive and diagnostic services are covered 100%; basic/minor restorative services and periodontics are covered 80%; and major restorative services are covered 50%. The plan’s deductible is $50 for an individual and $150 for a family plan. The calendar year maximum is $1500.

Our Safe Harbor 401 (k) plan is designed to assist you in your retirement and has a 4% maximum company matching amount that is 100% vested from the day you elect to contribute to it.
The maximum contribution that can be deferred during the 2017 plan year is $18,000 with additional $6,000 eligibility for those 50 and above, called a “Catch Up Contribution”. Compensation, for all qualified retirement plans, is limited to $265,000 for 2017.
The company will make safe harbor matching contributions equal to a) 100% of your pre-tax contributions that do not exceed 3% of your annual compensation and b) 50% of your pre-tax savings contributions that exceed 3% of your compensation but do not exceed 5%. The company matched contribution is made only after the tax year has been finalized.
