Contract Employee Benefits

<h4>Pay Period</h4><p>The work week begins on Sunday and ends on Saturday. We implement an electronic timecard system where employees enter their time online, and managers can approve timecards online. A timecard with a manager&rsquo;s signature must be received before a paycheck can be released. Electronic timecards must be submitted by the end of the workweek. The payroll deadline for approvals is 10:00am every Monday.</p><h4>&nbsp;</h4><h4>Direct Deposit</h4><p>Contract employees can sign up for direct deposit at the time of hire.</p><h4>&nbsp;</h4><h4>Workers&rsquo; Compensation</h4><p>An employee may be eligible for state workers&rsquo; compensation benefits for job-related injuries. If an employee is injured while performing a job-related duty, the employee should immediately report the accident to his/her supervisor. If the injury results in an absence from work, a letter from a doctor stating that the employee is physically well enough to return to work is required prior to, or on, the date of return.</p><h4>&nbsp;</h4><h4><strong>Section 125 Cafeteria Plan</strong></h4><p>This IRS-approved plan allows Lighthouse employees to have weekly medical insurance premiums deducted from their paychecks pre-tax. Your status, Full-time or Variable Hour, is determined when you are hired and is based upon the average number of hours you are expected to work over a period of time.</li><li>Full-time status: If you are expected to work on average at least 30 hours or more per week for six consecutive months when you are hired you are considered Full-time. Full-time employees are eligible for Major Medical and MEC after a 30-day waiting period from their date of hire.</li><li>Variable Hour status: If it cannot be determined that you are expected to work on average at least 30 hours or more per week for six consecutive months when you are hired you are considered Variable Hour. To become eligible, Variable Hour employees must work 780 hours in a 6-month look-back period. Meaning, you must have been employed by Staffing 360 Solutions for at least 6 months and have worked 780 hours within the last 6 months.</li></ul><h4>&nbsp;</h4><h4>Staffing 360 Solutions, Inc., and Subsidiaries 401(k)</h4><p>For the 2021 Plan Year, the Company will make a safe harbor matching contribution. The annual safe harbor matching contribution will be equal to 100% of your salary deferral contributions that do not exceed 3% of your compensation, plus 50% of your salary deferral contributions that exceed 3% of your compensation but do not exceed 5% of your compensation. The company-matched contribution for 2021 will be made by the end of 2022.</p><p>The advantage of 401(k) is that Staffing 360 has researched and carefully considered all the options in order to provide you with high-quality investment choices. You get to decide about the allocation plan that makes you comfortable. Once you have determined how your 401(k) account will be invested, your savings will be automatically invested as you have directed every time your contributions are deposited into the plan.</p><p>Participating in the Staffing 360 Solutions&nbsp;401(k) plan is the first and most important step on your journey to retirement. Once you are enrolled in the plan, you can manage your account and make periodic updates as your needs change.</p>

<p><strong>Professional Contract Assignments</strong><br />Our recruiters take you through the process offering resume suggestions, interview preparation, flexible job options, and competitive pay to place you in a great job with one of our clients. You then become a W-2 employee of Lighthouse Professional Services. Once placed, you are also eligible for our benefits package which includes health care options from <a href="https://www.lighthouseprofessionalservices.com/cm/158979">Aetna Health Insurance</a>.</p><p>&nbsp;</p><p><strong>Temporary to Hire Assignments</strong><br />Companies often utilize our service as a means to assess talent for direct-hire positions. These are situations where they have an immediate need, and the potential is there for a long-term relationship. This method gives you the opportunity to get your foot in the door at a great company and explore the possibility of direct hire employment.</p><p>&nbsp;</p><p><strong>Direct Hire Placement</strong><br />First, we get to know you. We keep your qualifications in our database and update your present job search status. Should the perfect opportunity become available that fits your needs, we make you aware of the opportunity. Often these positions are not widely advertised so there is less competition. We offer career counseling, resume review, and interview preparation, all to ensure that both parties are well served by Lighthouse Professional Services.&nbsp;&nbsp;</p>

Timecards & Pay

<p><strong><span lang="EN-US">Getting Paid</span></strong></p><p><span lang="EN-US">One of our highest priorities is paying you quickly.&nbsp; You are responsible for promptly and accurately submitting your time at the end of your workweek.&nbsp;</span>As your employer, Lighthouse deducts required federal, state, and city taxes, Social Security (FICA), and other legally required deductions (e.g., garnishments and tax levies) that have been served to Lighthouse.&nbsp;</p><p>You can view your paystub by&nbsp;<a data-cke-saved-href="https://apply.staffing360solutions.com/Account/Login" href="https://apply.staffing360solutions.com/Account/Login">clicking here</a>&nbsp;and accessing&nbsp;<a data-cke-saved-href="https://apply.staffing360solutions.com/Account/Login" href="https://apply.staffing360solutions.com/Account/Login">our portal</a>.</p><p><span lang="EN-US"><strong>Submitting Your Time</strong></span></p><p><span lang="EN-US">Lighthouse employees must record all actual hours worked, no more and no less.&nbsp; If instructed not to record or report all actual hours worked, for any reason, you must immediately notify your recruiter.&nbsp; Never report time for another employee.</span></p><p><span lang="EN-US">Lighthouse customers may use a variety of timekeeping systems (e.g., paper timecards, card swipe systems, web-based systems) but most of our clients use our electronic timekeeping system, TimeWorksPlus.&nbsp; Please follow the instructions for submitting your time as explained by your Lighthouse recruiter.&nbsp; Ensuring that your time is approved by the customer is your responsibility.&nbsp; Unapproved, inaccurate, or illegible timecards may result in the delay of your pay.</span></p><p><span lang="EN-US">If you have a payroll-related question, please contact the payroll department at&nbsp;<a data-cke-saved-href="mailto:lighthouse_payroll@staffing360solutions.com" href="mailto:lighthouse_payroll@staffing360solutions.com">lighthouse_payroll@staffing360solutions.com</a>.</span></p><p><span lang="EN-US"><strong>Calculation of Overtime and Regular Rate Overtime Adjustments</strong><br />Unless specifically exempted, an employee covered by the Fair Labor Standards Act (FLSA) must receive overtime pay for hours worked more than 40 in a workweek (or as otherwise legally required) by state-specific law at a rate not less than one and one-half times his or her regular rate of pay.</span></p><p><span lang="EN-US"><strong>Holiday/Vacation</strong><br />Lighthouse and its clients are not required to pay holiday or vacation pay.&nbsp; If you choose to work on a holiday, enter the hours on your timecard and you will be paid at a straight time.&nbsp; DO NOT enter &ldquo;Holiday&rdquo; or &ldquo;Vacation&rdquo; on your timecard.&nbsp; Enter a &ldquo;0&rdquo; (zero) or leave it blank.</span></p><p><span lang="EN-US"><strong>Sick Pay</strong><br />Contract employees employed by Lighthouse Professional Services and physically working for clients in Massachusetts will be eligible for Paid Sick Time.&nbsp; Lighthouse is in compliance with the M.G.L. c. 149, &sect; 148C; Earned Sick Time Regulation.</span></p><p><span lang="EN-US"><strong>Tax Withholding</strong><br />If you have questions&nbsp;or want to make changes to your Federal and/or State withholdings please direct all inquiries to the Payroll Department, and you will be required to complete a new W-4 Form.</span></p><p><strong><span lang="EN-US">Direct&nbsp;Deposit&nbsp;</span></strong></p><p><span lang="EN-US">With direct deposit, your pay is electronically deposited directly into your checking or savings account. To enroll, fully complete the Direct Deposit Authorization form in the onboarding portal.</span></p>

A Message to Our Contractors and Clients 

<p>With the recent events surrounding COVID-19, more commonly known as Coronavirus, we have outlined below our approach for contractors and clients.</p><p>We are working closely with our clients to monitor the developing situation, and we are assured that like us, our clients hold their main priority as the health and safety of their employees, agency workers and independent contractors. &nbsp;We have confirmed that our clients are seeking and following guidance from trusted health organizations to ensure that there is proper protocol in place during this concerning time.</p><p>We would encourage our contractors to understand the approach that each client will take in relation to the Coronavirus.&nbsp; If they are required to provide services to a site internationally in an affected area or are required to travel across multi sites in this country, we are encouraging them to ensure that they familiarize themselves with where to obtain the relevant Coronavirus information from the client contact.&nbsp;&nbsp;</p><p>We will take clear direction from our clients in respect to their working arrangements during this period, in the event that a client site needs to close as a result of suspected Coronavirus exposure, we ask contractors to take direction from the relevant client site contact with regards to ongoing service provision.&nbsp;</p><p>Unless we are notified differently by the client, we will process payment against services provided and approved by the client, as is currently the case.&nbsp; Obviously, for clarity, unless otherwise agreed by the client, payment will not be made in the event that the client suspends service for a period.</p><p>For those that have travelled to an at-risk location or feel that they are experiencing the symptoms of the Coronavirus, they are encouraged to:</p><ul><li>Self-quarantine for 14 days and comply with the client&rsquo;s published protocols.&nbsp; They should also let their recruitment consultant know.</li><li>Entitlement to any sickness payments will be dependent on a contractor&rsquo;s employment status, contractual terms, or the regulations in their locality.
